Why not join our new team which brings together the best of acute hospital and community care – enabling people to complete their hospital care at home or in many cases, avoid an hospital admission.

We provide safe, high quality acute care, diagnostics and interventions in patients’ usual place of residence. The @home service is at the forefront of healthcare delivery in providing acute care in the community. Working as a key member of the multi-disciplinary team and with Community and Primary Care services across Lambeth and Southwark, St Thomas’ and King’s College Hospitals, you will help patients recover more quickly and comfortably, using advanced clinical skills and supervising and mentoring junior colleagues.

Patients will range from those requiring a single episode of care, e.g. post-operative, to people living with complex or long-term conditions, including the frail elderly, who benefit from short-term acute interventions that our service provides. We work with GPs, Hospital teams, Community Matrons, District Nurses, Therapy teams, and Social and Emergency care services. The service operates between 08.00hrs to 23.00hrs 7 days per week across shifts.

You will be enthusiastic, able to work autonomously using advanced nursing practice and clinical reasoning, and have skills and experience to bring to the team.
