Triage Nurse - Clinical Nurse Band 7 – 1 Year Maternity Cover, Rotational between Acute and Community.

Are you interested in becoming a key member of our new innovative @home Service? We are offering a fantastic career opportunity in an exciting, forward thinking environment.

We are based in the Community, working as part of St Thomas’ or King’s College Hospitals. You will work closely with acute wards, emergency care teams and @home GPs, Matrons and Therapists and be pivotal in identifying and assessing adult patients who will benefit from the care of our @home multi-disciplinary team. You will also be involved in triaging referrals into the service through the Unified Point of Access - a centralised point of referral management for the @home service and Enhanced Rapid Response.

The @home service provides high quality urgent care, acute treatment intervention and monitoring in the patient’s usual place of residence by supporting advanced discharge or preventing an unnecessary hospital admission or readmission. This will range from a single episode of care e.g. post-surgery through to patients living with complex and long term conditions, including the frail elderly, who may require periodic short term care to maintain the best possible quality of life at home. Integrated under the @home name, the service is based at the two acute hospitals, two community @home bases and a central management ‘hub’. The @home service works across Lambeth and Southwark working closely with Community and Primary care services, Rapid Response Therapy teams and Social Care.

You will have significant advanced clinical experience in an acute, rapid response, emergency, or similar setting at a senior level and be keen to lead, and implement change. You will need to be a flexible team member, have sound clinical knowledge, excellent communication and interpersonal skills and the ability to work autonomously as well as within the wider multi-disciplinary team.
