We are looking for enthusiastic full-time District Nurse Sister/Charge Nurses to join the District Nursing Team in Southwark and Lambeth. Candidates need to demonstrate their ability to deliver high quality nursing care to patients with complex as well as challenging needs. The post will offer successful candidates a valuable experience of proactively meeting the health needs of the local population of Southwark and Lambeth. You must be forward thinking and motivated, providing clinical case management to patients with long-term conditions and end of life care needs.

You must also be able to maintain the highest standards of quality care for your patients. You will manage a caseload of complex, high risk and supported self-care patients. You must demonstrate excellent communication and team working skills. You will receive training and opportunities to develop your skills and competencies. You will also have access to clinical supervision and appraisal.

This post will involve working shifts between 8am and 8pm Monday to Friday including some rotation weekends.
