Band 7

Our SPT’s Provide clinical leadership and management to an adult community Nursing team and be responsible and accountable for the care delivered to a generic caseload of patients ensuring the provision of a high standard of nursing care to patients and their families in the community.

They also have a clinical caseload and provide expert care to patients in Lambeth and Southwark.

Ensure that each member of the nursing team is competent, able to meet the full needs of their clients and operates to high standards.

Provide support to staff in the locality who are teaching, assessing and mentoring others

Provide support and ensure that nurses undertaking specialist practice and other post grad training are developing in their roles and practice.

Work with the PDN team and other senior colleagues to ensure that staff in the team have the skills and are competent to meet the needs of their patients now and in the future.

Actively manage and case manage patients with long term conditions and work towards preventing complications and keeping people out of hospital.

Provide comprehensive nursing care to a high standard including palliation and end of life care.

Responsible and accountable for all aspects of performance within the community nursing team effectively role modelling high standards of care, conduct and behaviour

Provide first line management of a district nursing team including sickness and absence management and dealing with performance and capability issues.

Undertake regular appraisal and performance review and develop a personal development plan for each staff member that meets the needs of the service.

Most posts tend to involve working with the complete age range, but some posts may be designated to work with specific client groups, such as the elderly.
