Integrated Care Clinical Pharmacist - Care Homes

We are looking for a dynamic and capable pharmacist to lead medicines optimisation and improve medicines related outcomes for vulnerable adults with multiple long term conditions in care homes across Lambeth and Southwark. Many of these patients will be frail, over 65 years, have complex co-morbidities and be taking a number of medicines.

You will join a team of experienced primary and secondary care pharmacists within Guys and St Thomas NHS Foundation Trust who, through a range of innovative initiatives, are leading medicines optimisation across Lambeth and Southwark.

You will be allocated a number of care homes with nursing in which you will be responsible for undertaking patient centred and outcome focused medication reviews. There will be the opportunity to take a lead on specific areas including medicines information, audit, prescribing issues and management of medications.

We are looking for UK registered pharmacists registered with the General Pharmaceutical Council (UK), with post graduate clinical qualification and proven ability to influence within multidisciplinary or integrated teams for older people. The post requires you to be enthusiastic, highly motivated, able to plan and manage your own time to meet the needs and goals of the service.

We are looking for 1 x permanent post-holder and 1 x 9-12 month fixed term position.
