Guys and St Thomas’ NHS Foundation Trust is at the cutting edge of exciting developments in the Neuro-rehabilitation pathway in South East London!

An exciting opportunity has arisen for a suitably qualified and experienced clinician to join the innovative South East London Neuro Navigation Service (SELNNS). The successful candidate will join an established team comprising four existing Neuro Navigators (Band 7), and two Clinical Managers (Band 8a) who provide strategic oversight and management of the service.

The SELNNS works collaboratively with patients, families and various professionals across agencies to ensure that patients with complex neuro-rehabilitation needs are accessing the right service at the right time for them. This is achieved through the application of expert neuro-rehabilitation clinical reasoning skills, combined with knowledge of acute and community services across South East London. The team in- reaches to various tertiary and local hospitals, as well as specialist neuro-rehabilitation units across the capital to support seamless transition of care between settings.

The position is Band 7, 37.5 hours per week and likely based within the boroughs of Lambeth and Southwark, however travel and working within the other South East London boroughs will be required. The team are based at Guy’s Hospital near London Bridge Station, however all staff within the team are supported to enable remote working.

We are looking for an Occupational Therapist, Physiotherapist, Speech and Language Therapist or Specialist Nurse with:

  • Strong neuro-rehabilitation skills and experience (ideally including acute, specialist units and community roles)
  • The ability to work autonomously within a South East London Borough
  • Excellent communication skills with a range of patients, professionals and commissioners
  • Enthusiasm, initiative, the drive to work individually and a willingness to work as part of a team
  • Strong IT skills, experience of service development, data collection and knowledge of relevant outcome measures

This position provides an excellent opportunity for consolidation of leadership and management skills, as you will work collaboratively with Clinical Managers to:

  • Take a strategic lead on the development and implementation of a collaborative care pathway for people with complex neurological disability, advising the CCG on future models of care
  • Provide expert opinion on the appropriateness of referrals for out of borough assessment / intervention
  • Take on a broader strategic role, networking and liaising with neurological rehabilitation services, particularly across South East London
  • Work in partnership with other health and social care providers, promoting an integrated approach to the management of complex neurological conditions
  • Promote the provision of high quality care to adults with complex neurological disability through research and evidence based practice

A working knowledge of the health and social care landscape of any of the South East London boroughs of Lambeth, Southwark, Lewisham, Greenwich, Bexley or Bromley is not essential, but would be an advantage.
