We are offering an exciting and challenging role which brings together the best of acute hospital and community care – enabling people to complete their hospital care at home and in many cases, avoid a hospital admission. We provide safe, high quality care, treatment and interventions in patients’ usual place of residence and our @home service is at the forefront of healthcare delivery.

You will work as a key member of the multi-disciplinary team and with Community and Primary Care services across Lambeth and Southwark, Guys', St Thomas’ and King’s College Hospitals, including GPs, Hospital Teams, Community Matrons, District Nurses, Therapy Teams, London Ambulance Service, and Social and Emergency Care Services.

The service operates a seven day service between the hours of 08.00hrs to 23.00hrs. As a Physiotherapist you will be working from 08:00 to 20:00, covering 7 days a week on a rotational basis (37.5hrs/week).

In your role you will use advanced clinical assessment and reasoning skills to manage complex physiotherapy and cross professional issues, facilitating patients to recover more quickly and comfortably in their own homes. Patients will range from respiratory, medical or post-operative, to people with an exacerbation of complex or long-term conditions, including the frail elderly, who benefit from short-term acute interventions that our service provides.

You will be enthusiastic, flexible and able to work autonomously using strong clinical assessment and clinical reasoning skills. You will have supervisory, teaching and training responsibilities and the service is committed to supporting your development. We will offer you supervision from the Therapy Lead as well as a wide range of opportunities to develop your clinical practice, and opportunities to develop or consolidate your medical, nursing and multidisciplinary knowledge and skills.
