Guy's and St Thomas' NHS Foundation Trust has an excellent reputation of providing high quality innovative patient care. The Transplant, Renal & Urology Directorate are looking for a highly motivated Clinical Nurse Specialist to join our Advanced Kidney Care (pre-dialysis and CKD) clinic. The Renal Unit provides specialist care to renal patients to the local community and also extending to many parts of South East England. Our services include a leading living donor transplant centre and dynamic kidney transplant programme, a large home dialysis programmes and community based satellite dialysis units and kidney treatment centres.

Our Advanced Kidney Care Clinic focuses on care for patients with early CKD through to planning and preparing for dialysis, transplantation and supportive care. These multidisciplinary clinics are held at the Guy's hospital site and also in our outreach clinics at Sidcup and Tunbridge Wells Kidney Treatment Centres. The Kidney Clinic and inpatient wards are supported by our Rapid Assessment and Day Case Unit.

As an Advanced Kidney Care coordinator he/she will act as a lead specialist and will demonstrate a high level of expertise providing specialist advice, education and support to health care professionals, patients and carers. He/she will provide clinical expertise for patients in the pre- dialysis and pre- transplant treatment phase including education, social assessment and clinical monitoring.

The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines and contribute to service developments in collaboration with multidisciplinary colleagues both within hospital and community settings.

The successful applicant will have strong leadership and communications skills and will be able to demonstrate their ability to contribute to national and international issues related to chronic kidney disease and management of this group of patients.
