This new post within the KHP homeless team has been established to achieve two main things: to provide a high level of expert knowledge of housing law while confidently negotiating the systems and processes that affect housing and secondly to offer a leadership, support and supervisory role for the housing workers currently in the team.

The KHP team receive an ever increasing number of referrals for people in hospital who are either homeless or vulnerably housed. Following recent changes in legislation regarding the duty to refer, the Trust needs to demonstrate that we have a pathway for homeless people. The clinicians in the team will work alongside you to ensure that the team fulfils its remit to improve the health outcomes of homeless people in hospital.

This leadership role will require you to work with housing workers from voluntary sector organizations, therefore it is expected that you contribute to their training and development as well as to support them on a day to day basis with their caseload. Housing workers are in post across the Guys, St Thomas' and Kings Hospital sites and therefore you may work across site.

In turn, you will also be supported to take a leadership role for complex discharge cases for which the team now often need to seek legal guidance or to assert a patients entitlement to housing with housing departments and social services.

This will be a challenging role and you will need to have excellent verbal and written communication skills to enable you to liaise with a multidisciplinary team, work across the boundaries of the acute and community teams including statutory services and inform Trust managers of the blocks to progressing discharges for people who are considered homeless.
